What should an adult model release include?

At minimum: the performer's legal name and every name they use, date of birth and ID details, a certification they are 18 or older, explicit consent to the activities recorded, consent to distribution on named platforms and to download, payment terms, and a way to request removal.

Should a model release collect Social Security numbers?

It is safer not to. Collect tax details on a separate tax form stored securely and apart from the release, so the release can be shared with a platform or processor without exposing sensitive numbers.

Can a performer withdraw consent after signing?

Section 11 gives the performer a clear way to request removal, with acknowledgement and decision timeframes and a neutral third party for unresolved consent disputes. Card network rules for adult merchants expect a process like this.

Is a signed release enough on its own?

No. Keep it with a copy of the performer's ID, a photo of them holding it, an on-camera consent clip, and a 2257 record. See the ID verification checklist and the 2257 form.

11. My right to request removal

I may ask the Producer at any time to remove content I appear in by emailing [PRODUCER CONTACT EMAIL] or using the Producer's content removal request form. The Producer will:

(a) acknowledge my request within [1] business day;

(b) disable the content from public view while my request is reviewed if I say I did not consent or that it shows more than I agreed to;

(d) if a dispute about consent cannot be resolved, refer it to an independent neutral third party at the Producer's cost.

I will never be charged a fee, or asked to buy content or give up rights, as a condition of having my request reviewed or acted on.

12. Records and privacy
